Here are some helpful forms and sheets that may make your life easier.

Medical Release Form
Prior to your first class session your instructor will need to have from you a signed Medical Release Statement.
Please print this form and fill out each blank with a full 'YES' or 'NO'.
If you answer 'YES' to any of the questions please have your personal physician sign off on the form. Keep in mind that a 'YES' does not necessarily preclude you from Scuba Diving, it just means that your physician should be consulted in order to determine the safety of your participation. We are unable to advise you about what you should answer (we're Scuba Instructors - not Doctors), if you have any questions please discuss them with your doctor.
We can't put you in the pool without a cleared Medical - if you have no YES's your signature is all we need.

Release required for all dives and activities lead by Underwater Sports
This is the form you will need to have filled out prior to participation in any UWS sponsored trip, event or dive.
You will need to turn in a seperate form each time you go out on a dive with us.
You may also want to carry a copy of this release when going boat diving as it is fairly standard and can save you time at the dock. We suggest that you put a few extra copies in your log book so you'll always have them if you need them.


Get a Rx Scuba Mask.
Do you need corrective lenses for your mask? This form will walk your eye doctor or vision clinic through the information that we need to get you a Rx mask that will work for you.
Please make sure that they fill out all the areas that have YELLOW highlights.

Surface Air Consumption calculations made easy. This form will walk you through your SAC rate calculations.
This form will walk you through figuring out your Surface Air Consumption Rate.
Your SAC rate will allow you to figure out how much gas you will require to safely conduct a planned dive. This calculation is a very important part of advanced and technical dive planning.
Your SAC rate will change over time so we recommend that you figure out and record your SAC rate for every dive, this way you have a running standard for what your actual SAC rate should be.

Check list so that you make sure you don't forget to bring or bring back any of your scuba equipment.
Make sure you don't forget anything! This checklist is a handy reminder for all the gear you need to bring, and the information you need to have.
It has a convenient place to keep track of your gear as you pack it to go... and pack it to leave from the site.
The checklist also has places for information about the closest medical facilities to the site, ferry times, and tide information. This is the sheet we use when we're planning dives.

Calculations for Nitrox - made easy! MOD, PO2, EAD.
This sheet is designed to help you figure out the basic calculations required to safely plan Enriched Air Nitrox dives.
It shows easy ways to figure for 'Best Mix', 'MOD' and PO2.
