Forms & Medical History

Here is a copy of our medical form- Diver Medical Participant Questionnaire

Under the forms section you will find all documents that are needed to be filled out. ALL DOCUMENTS MUST BE COMPLETED PRIOR TO CLASS! Please follow the directions on the form to determine if you will need a physician’s signature prior to your SCUBA certification class. If a physician’s approval is required and you are not able to obtain one prior to the first day of class, we will need to reschedule your class for a later date.

Prerequisites

You will be required to complete a swim test. A 200-yard freestyle swim or a 300-yard mask, snorkel, and fin swim (decided by the instructor) plus a 10-minute tread must be completed to obtain the certification.

Equipment & more

Your course tuition includes the use of all SCUBA equipment necessary for your confined (pool) and open water sessions except for your personal snorkeling equipment (mask, snorkel, fins, boots, and gloves).  There are student packages available at all of our locations for this gear.  We will insure you are properly fitted and we offer a 100% satisfaction guarantee.  You may also find these packages on our e-commerce site. Bring your personal gear (removed from its retail packaging) and in a mesh bag or duffel bag. Be sure to bring a swimsuit and towel. You will also need to purchase a PADI logbook to log your training sessions and open water dives.

Your gear (kit) for Open Water class must be picked up the Thursday or Friday prior to your check out dives. Please allow 45-60 minutes for the gear rental process, as we want to make sure all rental gear is checked by the customer and staff together.

For any changes to your class—such as cancellations, rescheduling, or questions—please contact the Underwater Sports location where your class is scheduled.
